29 CFR 1926.760(a)(1): Each employee engaged in steel erection activity who is on a walking/ working surface with an unprotected side or edge was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. (a) On or about December 2, 2021 and at times thereto, employees performing steel erection roofing activities while working on a pre-engineered metal building were exposed to an approximate 17 foot fall hazard.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of this section. The employer did not institute a training program and ensure employee participation in the program. (a) On or about December 2, 2021 at times prior thereto, employees were not trained in appropriate fall protection methods to avoid hazards associated with working at elevated work areas and exposed to an approximate 17 foot fall hazard.

29 CFR 1926.454(a)(3): The employer did not train each employee who performs work on a scaffold, by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards, including the proper use of the scaffold and the proper handling of materials on the scaffold: (a) On or about December 2, 2021, and at times prior thereto, employees climbed the guardrails of a Skyjack scissor lift, model SJ111 3219 to access and/or egress from a steel roof structure and exposed to an approximate 17 foot fall. Workers had not received training on the safe use of the scissor lift, including, but not limited to the manufacturer's warnings to: "Use only equipped access openings;" "Enter and exit only when the aerial platform is in the fully retracted position;" and "Enter and exit the platform from the ground only."
