29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  JOB SITE: 1365 Park Ashwood Drive, Saint Charles, MO 63304, on or about 12/2/2021, workers were observed on the top of an 8:12 pitch roof,  approximately 26 feet 10 inches above ground level, actively removing old roof shingles, and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure.  The workers were exposed to fall hazards.

29 CFR  1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in mounting the ladder:  JOB SITE: 1365 Park Ashwood Drive, Saint Charles, MO  63304, on or about 12/2/2021,  workers were observed on a Werner Aluminum Extension Ladder, MODEL: 1328-2, 250lb Capacity, the ladder side rails did not extend at least 3 feet (.9 m) because of the ladder's length. The ladder was not secured to a rigid support, and a grasping device, such as a grabrail, was not used to assist the employees in mounting the ladder. The workers were exposed to a fall hazard.
