29 CFR  1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units.   On or about December 7, 2021, at a residential construction site located on 21110 Hill Summit Ct. in Cypress, TX, employees were working on the second level of tubular welded frame scaffold that was not fully planked and inadequate spacing with the guardrail supports,  exposing the employees to a fall hazard of more than 10 feet.

29 CFR  1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling. Note:  See exceptions in paragraphs (b)(3)(i) and (ii) of this section.   On or about December 7, 2021, at a residential construction site located on 21110 Hill Summit Ct. in Cypress, TX, employees were working on the second level of a tubular welded frame scaffold where the front edge of all platforms was not 14 inches (36 cm) or less from the face of the work and no personal fall arrest systems was used,  exposing the employees to a fall hazard of more than 10 feet.

29 CFR  1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates and mud sills or other adequate firm foundation:   On or about December 7, 2021, at a residential construction site located on 21110 Hill Summit Ct. in Cypress, TX, employees were working on the second level of tubular welded frame scaffold, did not have an adequate foundation for the baseplates and mud sills that would prevent the scaffold from settling into the ground.
