29 CFR  1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.   (a) At Chelsea Park Subdivision, Carnaby Street, Lot 232, on or about December 7, 2021 workers engaged in roofing work were  exposed to approximately a 20-foot fall hazard in that the employer allowed the use of rock climbing-type harnesses on the 5/12 pitched roof.

29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators.                                                                    (a) At Chelsea Park Subdivision, Carnaby Street, Lot 232, on or about December 7, 2021 workers were exposed to fall hazards of up to 20-feet to the lower level where they used a material hoist as a ladder to ascend and egress the roof.

29 CFR  1926.552(a)(4):Hoisting ropes were not installed in accordance with the wire rope manufacturers' recommendations.  (a) At Chelsea Park Subdivision, Carnaby Street, Lot 232, on or about December 7, 2021 workers were exposed to struck-by hazards where the employer replaced the wire rope with a nylon climbing type rope on the materials hoist.
