29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards while performing residential roofing activities. This was most recently observed at 2401 Diane St, Papillion on December 9, 2021 where the employer had employees working on the roof of a residential home without fall protection where the ground to eave was over 16 feet.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause them to lose balance and fall:  The employer failed to ensure that employees did not carry objects up the ladder that could cause the employee to loose balance and fall. This was most recently observed at 2401 Diane St., Papillion, Nebraska on December 9, 2021 where the employer had an employee carrying a shingle remover up a ladder where the ground to roof eave was over 16 feet.
