29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: On or about December 9, 2021, employees were exposed to a fall hazard of approximately 25 feet while in the basket of an aerial lift and the employer had not ensured employees were tied off to the basket.
