29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:  631 North St (Rt 8A)., Dalton, MA: On or about December 8, 2021, an employee was observed working on a welded tubular steel scaffold that was not fully planked between the front uprights and the guard rail supports on the working level that employees were utilizing to perform work.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).

29 CFR  1926.451(g)(1)(vii):For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not  protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  631 North St (Rt 8A)., Dalton, MA: On or about December 8, 2021, an employee was observed working on a welded tubular scaffold more than 10 feet above the next lower level(ground) and was not protected from falling by either guard rail systems, safety net systems or  a personal fall arrest system.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).
