29 CFR  1926.25(a):  During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas:  On or about December 7, 2021, at the jobsite, the employer did not ensure the scrap lumber with protruding nails were cleared from the work areas during the course of construction.  This exposed employees to trip and impalement hazards.

29 CFR  1926.451(b)(2):  Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  On or about December 7, 2021, at the jobsite, the employer did not ensure the scaffold platform, where employees were working from, were at least 18 inches wide.   This exposed the employees to a fall hazard.

29 CFR  1926.451(g)(1):  Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:  On or about December 7, 2021, at the jobsite, the employer did not ensure that the job made scaffolds, where employees worked from, had guardrails installed and/or the employees were in use of fall protection.  This exposed the employees to a fall hazard greater than ten (10) feet to the ground below.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about December 7, 2021, at the jobsite, the employer did not ensure the employees, installing trusses to a residential building, were in use of fall protection.  This exposed the employees to a fall hazard greater than six (6) feet to the ground below.
