29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   Scaffolds were not provided with fall protection, in the following instances:  a) Porch Roof - Fall protection was not provided for the workers on the ladder jack scaffold, located approximately 20 feet above the ground. b) Main Roof - Fall protection was not provided for the workers on the roof bracket scaffold, located approximately 22 feet above the ground.

29 CFR  1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   Fall protection was not provided, in the following instances:  a) Porch Roof - Workers were not provided fall protection, while working on a residential roof 12 feet above the ground. b) Main Roof - Workers were not provided fall protection, while working on a residential roof 21 feet above the ground.
