On or about December 15, 2021 and times prior thereto, the employer and site Superintendent did not initiate or maintain programs to ensure all workers in the performance of the contracted work were free from recognized hazards and did not enforce or correct the deficiencies at the work site, such as but not limited to, workers performing work activities over 6 feet and exposed to fall hazards.
