29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) 115 Hess RD. Collegeville, PA., Roof: On or about December 10, 2021, an employee worked on the high-sloped, cross-gable roof of a two-story, newly constructed, residential structure without fall protection and was exposed to a fall hazard of not less than 20 feet.    (b) 115 Hess RD. Collegeville, PA, 2nd Level:  On or about December 10, 2021, an employee worked on the 2nd Level perimeter of a two-story, newly constructed, residential structure without fall protection and was exposed to a fall hazard of not less than 15 feet.

29 CFR  1926.1052(c)(1):Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stair-rail system along each unprotected side or edge:  (a) 115 Hess RD. Collegeville, PA.., 2nd Level:  On or about July 23, 2021, employees access the second level of a residential structure under construction by using stairs that were without handrail.
