29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On or about December 20, 2021; west side and rear area of residential site; employees working near the scaffold system were not wearing hard hat to protect them from falling objects such as but not limited to tools or materials used in the process.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.451(b)(1):Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights   a) On or about December 20, 2021; West-side and rear area of the residential site; the cross-over platforms of the scaffold system were not fully planked exposing the employees to an estimated 25-foot fall hazard while conducting masonry or traveling between scaffolds.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) On or about December 20, 2021; West-side and rear area of the residential project; employees conducting masonry work were exposed to an estimated 25-foot fall while walking across an unguarded cross-over which united the scaffolds and while working on the scaffold system.
