This was most recently documented on December 29, 2021, at a residential construction site located at 600 SW Hillside, Grain Valley, MO. An employee was not wearing protective helmet while working in a trench below struck-by hazards of debris from the spoil pile.

The employer is failing to protect employees from struck-by and collapse hazards associated with trenching activities. This was most recently documented on December 29, 2021, at the residential construction site located at 600 Hillside Dr, Grain Valley, Missouri. Employees were working in a trench with the spoil pile located at the edge of the trench.

An employee was working in a 9 foot deep trench with no cave-in protection.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  The employer is failing to protect employees from fall hazards associated with access and egress from a trench. This was most recently documented on December 29, 2021, at the residential construction site located at 600 SW Hillside, Grain Valley, Missouri. Employee was exposed to fall hazards in that the portable ladder used for access/egress of the trench did not extend 3 feet above the top of the trench. The ladder used was a Werner 6 foot step ladder, and the depth of the trench when the deceased was recovered was approximately 9 feet deep.

This was most recently documented on December 29, 2021, at the residential construction site located at 600 SW Hillside Dr, Grain Valley, Missouri. Employees were using 6' portable step ladder that was placed in the trench upside down, in the closed position leaning against the north end of the trench that was 8'9" deep.
