29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  The employer is failing to protect employees from fall hazards while performing residential roofing activities. This was most recently observed at 11854 S. 112th St., Papillion, Nebraska on December 28, 2021 when the employer has employees using a ladder whose side rails did not extend at least 3 feet past the roof eave and the ladder was not secured.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards while performing residential roofing activities. This was most recently observed at 11854 S. 112th. St., Papillion, Nebraska on December 28, 2021 where the employer has employees installing roof materials on a new residential home without fall protection where the ground to eave was 11 foot.   Calzada Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13)),  which was contained in OSHA inspection number 1524381, citation number 1, item number 1 and was affirmed as a final order on July 6, 2021, with respect to a workplace located at 6990 N 172nd St, Bennington, Nebraska.
