29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about January 4, 2021, employees were exposed to a stuck by hazard while performing roofing activities on a residential home located at 1222 Ledgview Drive, Hinckley, Ohio 44233.  An employee was working above a lower employee with tools and materials that could have fallen and the employer did not ensure that the lower employee was wearing a protective helmet.

An employee was using an air powered nail gun and the employer did not ensure that they were wearing protective safety glasses.

Employees were working on all areas of the roof and the employer did not ensure that they were wearing fall protection.

Employees were using an extension ladder to access the roof and the employer did not ensure that it extended at least three feet above the roof line.
