29 CFR  1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft:   a) Jobsite located at 1393 W Baltimore Pike, Media, PA: Full-time and contract employees were exposed to entanglement hazards while working in close proximity to several gravity filling systems located throughout the facility, in that projecting horizontal shaft ends did not have smooth edges and/or were not guarded by nonrotating caps or safety sleeves.   Condition noted on or about the 6-month period of June - December 21, 2021.

29 CFR  1910.219(e)(1)(i): Horizontal belts which had both runs at 42 inches or less from the floor were not fully enclosed in accordance with paragraphs (m) and (o) of this section.   a) Jobsite located at 1393 W Baltimore Pike, Media, PA: Full-time and contract employees were exposed to caught-in between hazards while working in close proximity to several gravity filling systems located throughout the facility, in that at least two horizontal belt assemblies did not have a guard installed.   Condition noted on or about the 3-month period of Sept. - Dec. 21, 2021.
