29 CFR  1926.404(b)(1)(iii)(C):Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects., such as deformed or missing pins or insulation damage, and for indications of possible internal damage.  On or about January 7, 2022 at this workplace employees conducting construction operations employees utilized a damaged 3-wire grounding single standard electrical outlet adapter with a broken grounding prong  supplying  power to a 120vac portable  construction light and a power tool battery charger via an electrical extension cords.  The employer did not visually inspect  before each day's use for  external defects and missing grounding pin/prong.
