29 CFR  1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position.  Location - 1640 Main Street, Concord, Ma.  On or about January 10, 2022:  a) An employee climbed out of an aerial lift basket at a height greater than sixteen feet above lower level (frozen ground).  b) An employee was straddling (sitting) on the top rail of an aerial lift basket at a height greater than sixteen feet above lower level.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  Location - 1640 Main Street, Concord, MA.  On or about January 10, 2022, an employee was not protected from falling while working/walking on a roof greater than sixteen feet above lower level.

Location - 1640 Main Street, Concord, MA.  On or about January 10, 2022, personal fall arrest system components were not inspected as required, and/or defective components were not removed from service. -
