29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee use appropriate eye / face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a.  On or about January 10, 2022, at the north side of Building #3, the employer exposed employees to eye injuries by flying debris, in that one employee using a pneumatic nail gun to nail roof  trusses was not wearing safety glasses.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a.  On or about January 10, 2022, at the north side of Building #3, the employer exposed employees to a 10 ft. fall hazard to the floor below, in that 2 employees were installing floor decking while walking/standing on the floor joists without a fall protection system.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a.  On or about January 11, 2022, between the 2nd and 3rd floor of Building #3, the employer exposed employees to a 10-feet fall hazard, in that employees doing framing work on the 3rd floor of the building used a job-made ladder that had a broken side rail and did not extend 3-ft above the landing surface.
