29 CFR  1904.4(a):The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent:  a) On or about January 4, 2022, an employee  exposed to COVID-19 in the workplace tested positive for the COVID-19 virus and was quarantined. The illness was not recorded on the injury and illness logs.  b) On or about January 4, 2022, an employee exposed to COVID-19 in the workplace tested positive for the COVID-19 virus and was quarantined. The illness was not recorded on the injury and illness logs.  c) On or about January 9, 2022, an employee exposed to COVID-19 in the workplace tested positive for the COVID-19 virus and was quarantined. The illness was not recorded on the injury and illness logs.  d) On or about January 19, 2022, an employee exposed to COVID-19 in the workplace tested positive for the COVID-19 virus and was quarantined. The illness was not recorded on the injury and illness logs.
