29 CFR  1926.651(k)(1):An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:  a) 159 Lexi Lane, Baldwinsville, NY  13027, on or about 1/12/22:  Employees were inside a trench, installing the connection from the house to the main, while inside an unprotected trench, with vertical cuts in portions, no safe egress from the trench, and without a competent person able to describe the correct soil type they were they were working.
