29 CFR  1926.501(b)(4)(i): The employer failed to ensure each employee on walking/working surfaces were protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  (a)  Old Tire Building - On January 10, 2022, and times prior, the employer did not protect employees working near skylights from falls greater than 6 feet, thereby exposing employees to a fall hazard.
