29 CFR  1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports:   a) Jobsite - 800 Lancaster Avenue, Berwyn, Pennsylvania - Employees working from a fabricated frame scaffold, approximately 5 feet high, 7 feet long and 5 feet wide, where working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports, thus exposing workers to fall hazards.  Observed on or about 05 JAN 2022.

29 CFR  1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  a) Jobsite - 800 Lancaster Avenue, Berwyn, Pennsylvania - Employees were working from a fabricated frame scaffold, approximately 5 feet high, 7 feet long and 5 feet wide, where the vertical uprights did not bear on base plates thus exposing workers to fall hazards.  Observed on or about 05 JAN 2022.
