This was most recently documented on or about January 18, 2022 at a residential construction jobsite located at 19818 Briar St., Gretna Nebraska. Employees were operating pneumatic nail guns and portable saws while engaged in residential framing activities without the use of eye protection.

The top level of a 8-foot stepladder was used by employees to egress from the upper landing surface of the residential structure.
