29 CFR  1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  Worksite: When an employee accessed a scaffold to conduct work, the scaffold was not fully planked to protect employees from a fall in excess of 10 feet to the ground below.

29 CFR  1926.451(b)(4): Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15 cm):  Worksite: When the ends of scaffold platforms were extended less than 6 inches over the centerline of its support, the employer did not have their ends cleated, restrained, or otherwise secured when they were accessed by workers conducting roof sheathing operations.

29 CFR  1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level:  Worksite: When an employee was conducting roof sheathing tasks in excess of 10 feet above ground level, the worker did not wear adequate fall protection that would prevent a fall to the ground below.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   Worksite: An employee accessed a roof from the scaffold pic to conduct roof sheathing operations and was not protected against falls by guardrail systems, safety net systems, personal fall arrest systems, or equivalent protection.  While on the roof the worker fell to the ground below and sustained serious injuries.
