29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a. On or about 01/19/2021, at the job site - employees were exposed to mechanical hazards while conducting maintenance or repairs to the conveyor system, in that, the employer did not establish a program consisting of energy control procedures, training and periodic inspections exposing employees to hazards related to unexpected energizing, startup or release of stored energy.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a. On or about 01/19/2021, at the job site - employees were exposed to caught-in and crushed-by hazards, in that, the belt  conveyor system is lacking machine guarding along the bottom areas and each open sides of the conveyor system.

29 CFR  1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:   a. On or about 01/19/2021, at the job site - employees were exposed struck-by hazards, in that, the employers did not require the use of head protection while working near or under the cement mixing hopper line where rocks fall to the lower-level ground.
