29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about January 20, 2022, and days prior, employees working on and around the Kent Owens Angle Ring Cutter (Machine No. 1-M), were exposed to an amputation hazard. The employer did not ensure the unused portion of the saw blade was properly guarded.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about January 20, 2022, the safety interlocks were not properly working on the Puma 240 CNC machine (Model No. 240LB and Serial No. PM240522). This condition allowed employees to operator the machine with the doors open, which exposed employees to an entanglement hazard. (b) On or about January 20, 2022, the safety interlocks were not properly working on the Puma 250 CNC machine (Model No. 250B and Serial No. PM250221). This condition allowed employees to operator the machine with the doors open, which exposed employees to an entanglement hazard.
