29 CFR  1926.21(b)(2):The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  A.) On or about January 19, 2022, and prior to that date, employees were not adequately trained in the recognition and avoidance of hazards associated with the installation of new natural gas fueled boilers in that while installing a new boiler there was a natural gas explosion.
