29 CFR 1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a) 1671 Piedmont Drive, Auburn, AL 36831; On or about January 19, 2022 and at times thereto, the employer exposed employees to struck-by hazards in that employees were not provided a hard hat while working on the ground beneath employees on the roof utilizing nail guns, hammers, hand tools and roofing materials which could have resulted in contusions, lacerations and broken bones. b) 806 Greenwood Drive, Auburn, AL 36830; On or about January 18, 2021 and at times thereto, the employer exposed employees to struck-by hazards in that employees were not provided a hard hat while working on the ground beneath employees on the roof utilizing nail guns, hammers, hand tools and roofing materials which could have resulted in contusions, lacerations and broken bones.

29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a) 1671 Piedmont Drive, Auburn, AL 36831; On or about January 19, 2022 and at times thereto, the employer exposed employees to eye injuries in that the employer did not provide safety glasses while employees operated pneumatic nail guns to install shingles over wood decking.

Mr. Greg Dykes, current owner of Superior Roofing of Russell County, LLC who previously owned/used the company name DGS Dykes, LLC and was previously cited for a violation of this or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1413445, citation number #1, item number #1 and was affirmed as a final order on November 12, 2019, with respect to a workplace located at 5162 Whitesville Road, MED Care, Columbus, GA 31904.
