29 CFR 1910.1200(e)(1):  The employer did not develop, implement, and maintain a written hazard communication program which at least described how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also included a hazardous chemicals list and methods used to inform employees of the hazards of non-routine tasks.  (a)	PRG 2 LLC DBA Fainting Goat at 846 Broadway, Denver, CO 80203:  On and before January 26, 2022, the employer did not develop, implement, and maintain a written hazard communication program which at least described how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also included a hazardous chemicals list and methods used to inform employees of the hazards of non-routine tasks. This condition exposed the employees to hazards associated with cleaning and sanitizing chemicals, such as, but not limited to:  (1)    Glass cleaner (2)    Toilet bowl cleaner (3)    6% hypochlorite bleach (4)    Multi-surface cleaner (5)    Heavy duty degreaser (6)    Mold stain and mildew stain remover

29 CFR 1910.1200(h)(1): The employer did not provide employees effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a)	PRG 2 LLC DBA Fainting Goat at 846 Broadway, Denver, CO 80203:  On and before January 26, 2022, the employer did not ensure that employees were provided effective training on hazardous chemicals such as, but not limited to:  (1)    Glass cleaner (2)    Toilet bowl cleaner (3)    6% hypochlorite bleach (4)    Multi-surface cleaner (5)    Heavy duty degreaser (6)    Mold stain and mildew stain remover

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):   (a)	PRG 2 LLC DBA Fainting Goat at 846 Broadway, Denver, CO 80203:  On and before January 26, 2022, the employer did not ensure through written verification that a workplace personal protective equipment (PPE) hazard assessment had been performed.  Abatement Note: The employer shall verify that the required workplace assessment has been performed through a written certification that shall include the following:      1)   Identifies the workplace evaluated;      2)   The person certifying that the evaluation has been performed;      3)   The date(s) that the hazard assessment was performed; and      4)   Which identifies the document as a certification of hazard assessment.
