29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   On the construction site, employees engaged in spray painting activities without wearing eye protection were exposed to eye injuries from paint particles.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).    On the south side of the construction site, employees performing painting activities without fall protection were exposed to fall hazards of approximately 9 feet to the ground below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.   On the south side of the construction site, employees using an extension ladder that did not have the side rails extended three feet over the upper landing surface were exposed to potential fall hazards greater than six feet.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On the construction site, the employer did not develop or provide fall protection training to the employees engaged in painting while working over 6 feet to recognize and eliminate hazards such as fall protection.
