29 CFR  1926.405(g)(1)(iii)(C): Flexible cords and cables were run through doorways, windows, or similar openings:    On or about February 1, 2022, and a times prior thereto, employees used an electrical extension cord ran thru a doorway up to the roof to supply electrical power to a circular saw used on top of the roof.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about February 1, 2022, and at times prior thereto, at least three roofing employees were observed stripping roofing from a preexisting rooftop without fall protection. The employees were exposed to a fall hazard of approximately twelve-foot six-inches to the concrete driveway below.

29 CFR  1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about February 1, 2022, and at times prior thereto, at least three roofing employees were observed stripping roofing from a preexisting rooftop without fall protection. The employees were not trained to recognize or install fall protection. The employees were exposed to a fall hazard of approximately twelve-foot six-inches to the concrete driveway below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about February 1, 2022, at least three roofing employees were observed stripping roofing from a preexisting rooftop. The employees used an extension ladder to access and egress the roof was not extended three feet. The employees were exposed to a fall hazard of approximately twelve-foot six-inches to the concrete sidewalk below.

29 CFR  1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  On or about February 1, 2022, and at times prior thereto, at least three roofing employees were observed stripping roofing from a preexisting rooftop. The employees used an extension ladder to access and egress the roof that was not extended three feet above the upper landing surface. The employees were not trained to properly set up a ladder and prevent ladder hazards. The employees were exposed to a fall hazard of approximately twelve-foot six-inches to the concrete sidewalk below.
