29 CFR 1910.36(g)(2):An exit access must be at least 28 inches (71.1 cm) wide at all points. Where there is only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge must be at least equal to the width of the exit access. On or About February 02, 2022, the employer did not ensure the access to the exit doors was at least 28 inches wide while working in the warehouse. Material was stacked throughout the warehouse which eliminated the employee's ability to access the exit doors in the southside fire corridor and in the northwest corner of the warehouse.

29 CFR 1910.176(a):Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked. On or about February 02, 2022, the employer did not ensure sufficient safe clearances allowed for aisles and passageways when mechanical handling equipment is used. Employees were exposed to stuck-by hazards as several high tiered stacks of material were leaning and/or collapsing within the aisles and passageways.

Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. On or about February 02, 2022, the employer did not ensure that material stacked on pallets throughout the warehouse were stable and secure against sliding and falling. Employees were exposed to stuck-by hazards as several stacks were leaning and/or collapsing.
