29 CFR  1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:   a. Bridgestone Americas Tire Operations, LLC dba GCR Tires & Service, 4500 E. 51st Ave, Denver, CO 80216: On and before January 10, 2022, the employer did not ensure that employees were protected from falls in that employees were directed to move tires from the second story mezzanine to the first floor. Employees are exposed to falls at heights of 10 feet. This condition exposed employees to broken bones and head injuries.

29 CFR  1910.28(c): When employees were exposed to falling objects the employer did not implement measures to prevent objects from falling from higher levels as set forth in (c)(1)-(3) of this section:  a. Bridgestone Americas Tire Operations, LLC dba GCR Tires & Service, 4500 E. 51st Ave, Denver, CO 80216: On and before January 10, 2022  the employer did not ensure that the walking-working area in the warehouse was free from falling objects.  An employee was struck by a tire after it was dropped from the second story mezzanine to the first floor while the employee was leaving the break area.  This condition exposed employees to  struck-by hazards.
