29 CFR 1926.303(d):All abrasive wheels and tools used by employees did not meet other applicable requirements of American National Standards Institute, B7.1-1970, Safety Code for the Use, Care and Protection of Abrasive Wheels: a) GRIC D3 MULTIPURPOSE BUILDING, ROOM E09: Employees cutting tile using a 4.5 inch Makita Angle Grinder Model GA4530, did not have the side grip handle in place, exposing them to cut and struck by hazards. b) GRIC D3 MULTIPURPOSE BUILDING, ROOM E08: Employees cutting tile using a 4.5 inch Makita Angle Grinder Model XYG25, did not have the side grip handle in place, exposing them to cut and struck by hazards.

29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) GRIC D3 MULTIPURPOSE BUILDING: Training on the hazards associated with the installation of tile on construction sites and the means to control those hazards was not provided to employees. Employees regularly used saws, lasers, grinders, and other electric powered hand tools, exposing them to laceration and health hazards.
