29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:   a. Excellent Roofing Limited at 20659 Austin Rd., Austin, CO 81410: On and before February 2, 2022, the exposing employer did not ensure that employees were protected from injury, in that three employees were observed working on the roof of CMU Softball Stadium at 908 Texas Ave Grand Junction, CO 81501. The employer did not protect employees against falls at heights over 20 feet. This condition exposed employees to head injuries and broken bones.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a. Excellent Roofing Limited at 20659 Austin Rd., Austin, CO 81410: On and before February 2, 2022, the exposing did not ensure that the portable extension ladder, used to access the roof of the Colorado Mesa University baseball field building had side rails extending 3 feet above the upper landing, and was secured. This condition exposed three employees to fall hazards of approximately 20 feet.
