Located at 1535 Timbertop Street in Lancaster, Ohio: On or about February 8, 2022, employees were observed working from the oriented strand board (OSB) sheathing stacked on the forks of a rough terrain forklift hoisted to the second story roof elevation. The employees were not protected from an approximately twenty-foot fall hazard while working from the makeshift platform.

In no case shall the original safety factor of the equipment be reduced.  a. Located at 1535 Timbertop Street in Lancaster, Ohio: On or about February 8, 2022, the employer was using an oriented strand board (OSB) stack on the forks of a JLG rough terrain forklift as a personnel platform for employees sheathing the roof trusses. The makeshift platform was not approved by the equipment manufacturer to hoist personnel.

Located at 1535 Timbertop Street in Lancaster, Ohio: On or about February 8, 2022, employees were performing sheathing work from the roof trusses without fall protection, thereby exposing employees to falls from approximately twenty feet.
