29 CFR  1910.151(b): In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which is used for the treatment of all injured employees, a person or persons were not adequately trained to render First Aid:   a) 589 Centerport Road, Mohrsville PA - The employer did not ensure that a person or persons were trained to render first aid where forklifts and other equipment, such as but not limited to a Universal Jumbo Press, are capable of causing serious injuries to employees, on or about February 2, 2022.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   1) Universal Jumbo Press - Machine guarding was not provided to prevent employee entry onto the catwalk exposing employees to ingoing nip points. An employee wiping ink off a roller was injured when their hand became caught in between the rotating rollers, on or about February 2, 2022.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.145(c)(2)(i): Caution signs were not used to warn against potential hazards or to caution against unsafe practices:   1) Universal Jumbo Press - There was no caution sign marked on the exterior of the machine or adjacent structures, so as to be seen by persons working in the area and to notify them of in-running nip point hazards created by rotating rollers, on or about February 2, 2022.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.145(d)(6): The standard color background of a safety instruction sign was not white and the panel, green with white letters. Any letters used against the white background was not black. The colors were not opaque glossy samples as specified in Table 1 of ANSI Z53.1-1967 or in Table 1 of ANSI Z535.1-2006 (R2011) incorporated by reference in 29 CFR 1910.6:  a) Universal Jumbo Press - A safety instruction sign posted on the support structure for the power brush was blue with white lettering, on or about February 2, 2022.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
