29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) On or before February 8, 2022, and at times prior, a third party worker was troubleshooting a CNC machine and was required to use a lockout/tagout on the CNC machine and was exposed to possible serious injuries.  Gulfco Manufacturing LLC did not establish an energy control program consisting of energy control procedures, employee and third party workers training and periodic inspections for their lockout/tagout. This condition exposed third party worker to serious hazards.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or before February 8, 2022, and at times prior, employees used a vertical band saw at the sheet metal cutting area. The adjustable guard on the exposed blade of the vertical band saw was missing. This condition exposed employees to possible serious hazards while using the band saw.  b) On or before February 8, 2022, and at times prior, employees used a vertical belt sander at the sheet metal cutting area. The guard cover on the belt and pulley of the vertical belt sander was missing. This condition exposed employees to possible serious hazards while using the belt sander.  d) On or before February 8, 2022, and at times prior, an employee was working on a tuna canning machine at the production area. The guard cover on the belt and pulley of the tuna canning machine was missing. This condition exposed employees to possible serious hazards while using the canning machine.
