29 CFR 1910.37(b)(2):Each exit was not clearly visible and marked by a sign reading "Exit": a) Warehouse Emergency Exit Door - On or about 02/04/2022, employees who worked in the Wearhouse area of Direct Flooring Source, Inc. were potentially exposed to fire hazards in that, an emergency exit door leading to the outside did not display an "EXIT" sign above it. The door is located next to three (3) rollup doors. It is only door used by the employees of Direct Flooring Source, Inc. to enter and exit the building. No other exit door was visible.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: a) Warehouse Area: On or about 02/04/2022, and at times prior, employees were exposed to struck-by and/or fire hazards, in that the employer failed to ensure that four (4) propane compressed gas cylinders were properly stored. Employees who worked in the proximity to unsecured gas cylinders were not protected should the cylinders fall or be struck-by a powered industrial vehicle and become a projectile.
