29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  On or about 2/8/2022 and 2/9/2022 at the jobsite located adjacent to  660 River St, Batesville, AR, employees were installing manhole concrete forms in an excavation.   The north wall of the approximate 10-foot deep excavation was not adequately sloped, exposing the employees to the hazard of being caught in the excavation in the event of a cave-in.
