29 CFR 1910.132(d)(2):The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done: On or about February 7, 2022, the employer did not ensure that a hazard assessment identifying and selecting appropriate personal protective equipment was conducted prior to engaging in hot work operations in the maintenance department. The employees performing non-routine tasks were exposed to physical hazards associated with maintenance operations, due to the creation of flying sparks and particles while using a DeWaltj angle grinder to cut the lid from a previously used drum.

29 CFR 1910.243(c)(3):Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel: On or about February 7, 2022, the employer failed to ensure the guard for the vertical portable grinder was located between the wheel and the operator. Employees were exposed to flying debris, to include sparks generated by the vertical portable grinder. Additionally, this exposes the employees to flammability hazards due the inability of the guard to properly deflect sparks and debris.

29 CFR 1910.1200(h)(1):Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about February 7, 2022, the employer did not adequately train employees on the chemical hazards present in their workplace. Employees were exposed to hazards associated with the usage of Ethanol, such as the flammability properties when exposed to an ignition source, such as cutting on a container that had previously contained liquid methanol in the maintenance area, which exposes employees to fire and explosion hazards.
