Plyler Enterprises, Inc. dba Plyler Entry Systems, Loading Dock #46, 2455 Robison Rd., Erie PA, on or about February 2, 2022, and times prior thereto, employees periodically conducted servicing, maintenance, and repairs to equipment including, but not limited to, McGuire brand MP-Series Dock Levelers.  The employer had not developed and implemented written energy control procedures, employee training or periodic inspections.

Plyler Enterprises, Inc. dba Plyler Entry Systems, Loading Dock #46, 2455 Robison Rd., Erie PA, on or about February 2, 2022, and times prior thereto, prior to beginning the servicing maintenance and repairs to the McGuire brand MP-Series Dock Leveler, the hazardous energy associated with the elevated dock plate was not rendered safe. While utilizing a Liftmoore Truck Crane, 6036DX-22 RB3 (P/N 23741) to lift and hold the dock plate to allow the employees to work underneath, the dock plate fell onto the employees, causing serious injuries.

Plyler Enterprises, Inc. dba Plyler Entry Systems, Loading Dock #46, 2455 Robison Rd., Erie PA, on or about February 2, 2022, and times prior thereto,  employees were conducting servicing maintenance and repairs to the McGuire brand MP-Series Dock Leveler utilizing a Liftmoore Truck Crane, 6036DX-22 RB3 (P/N 23741) to lift and hold the dock plate while the employees worked underneath.  The crane operator did not maintain a position in which the crane could be operated at all times.  During the maintenance operations, the dock plate fell onto the employees, causing serious injuries.
