Located at 255 Arlington Drive in Pickerington, Ohio: On or about February 10, 2022, employees were performing siding work from a pump jack scaffold platform that was supported on one end by direct placement on the roof brace. The improperly supported platform exposed employees to a fall hazard from approximately fifteen feet.

Located at 255 Arlington Drive in Pickerington, Ohio: On or about February 10, 2022, employees were performing siding work from a pump jack scaffold platform that did not have a complete fall protection system in place, thereby exposing employees to a fall hazard from approximately fifteen feet.
