29 CFR  1915.73(b):When employees were working in the vicinity of flush manholes and other small openings of comparable size in the deck and other working surfaces, such openings were not suitably covered or guarded to a height of not less than 30 inches.  a) Riverside Marine dba Hansen Industries on or about February 9, 2022, at the site location of Vessel Joseph H. Thompson (ballast tank #5) CSX dock 1 Oregon, OH: The employer did not ensure flush mounted 24"x14" opening used to access ballast tank #5 was guarded along vessel catwalk below deck. In this condition employees walking/working in proximity to the opening were exposed to fall hazards up to 91" to the hard surface below.

29 CFR  1915.73(c): When employees were working around open hatches not protected by coamings to a height of 24 inches, or around other large openings, the edge of an opening was not guarded in the working area to a height of 36 to 42 inches.  a) Riverside Marine dba Hansen Industries on or about February 9, 2022, at the site location of Vessel Joseph H. Thompson CSX dock 1 Oregon, OH: The employer did not ensure an open cargo hatch surrounded by a 18 inch coaming was adequately guarded while employees operated in the area. In this condition employees working in this area were exposed to fall hazards up to 30 feet to the hard surface below.
