29 CFR 1910.146(d)(6): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not provide at least one attendant outside the permit space into which entry was authorized for the duration of entry operations: On or about February 10, 2021, and times prior thereto, employer did not provide an attendant as employees conducted fire watch duties and accessed cable area room 215D, via the hatch door on the floor in room 318. employee was doing the fire watch task and exposed to fall hazard.

29 CFR 1910.23(b)(13):The employer did not ensure that no employee carries any object or load that could cause the employee to lose balance and fall while climbing up or down the ladder. On or about February 10, 2021, and times prior thereto, employees were exposed to a fall hazard while descending to a lower level and the holding hatch door open.

29 CFR 1910.305(b)(2)(i):All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. On or about February 10 , 2022 and times prior thereto, in office building, employees were exposed to electrical hazards in the workplace when a cover on the electric outlet was inadequate.
