29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a) Worksite:  On or about 2/14/2022, the employer did not adequately instruct each employee on the building plans regarding emergency lighting that needed to be de-energized, and lockout/Tag-out applied, to prevent an employee exposure to a hazardous electrical energy source.

29 CFR  1926.97(b)(2)(i): Protective equipment used for the primary insulation of employees from energized circuit parts was not capable of passing a current test when subjected to the highest nominal voltage on which the equipment is to be used:   (a) Worksite:  On or about 2/14/2022, the employer did not ensure that employees used electrical-insulating gloves while testing a 277-voltage power line to determine if it was de-energized.

29 CFR  1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  (a) Worksite:  On or about 2/14/2022, the employer did not ensure that employees were protected from electrical shock by de-energizing the circuit, and by using electrical-insulating gloves while testing a 277-volt power-line or another means of insulation to prevent an exposure to live electrical circuits.
