29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: a) YF Logistics LLC. - Warehouse behind Bay doors # 69-71: Boxes containing refrigerators were stacked up to 2 tiers, with an approximate height of 15 feet, the boxes containing the refrigerators were not secured or interlocked to prevent them from falling in any direction while being unloaded to the warehouse floor. On 02/15/2022 one of the refrigerator boxes fell onto an employee resulting in an inpatient hospitalization and multiple permanent injuries. b) YF Logistics LLC. - Middle of warehouse behind the FW Aisle racks: Boxes containing washing machines and drying machines were stacked up to 4 tiers, with an approximate height of 15 feet, the boxes containing the appliances were not secured or interlocked to prevent them from falling or collapsing in any direction, observed on or about 02/17/2022.
