When an employee is exposed to falling objects, the employer did not ensure that each employee wears head protection that meets the requirements of subpart I of this part and/or the employer did not protect employees from falling objects by implementing one or more of the following methods listed in 1910.28(c)(1)-(3).   a) Haverty's Pylon Sign Area (Airport Blvd Srvc Rd/Popeyes Restaurant Work Zone):  On or about February 23, 2022, the employer failed to provide head protection for ground employee(s) working underneath and in close proximity to an elevated aerial lift platform performing electrical service/maintenance operations; exposing ground employee(s) to struck-by hazards from falling/flying equipment, and materials such as ballasts,  fluorescent light bulbs and the bottom of the platform.  b) Haverty's Pylon Sign Area (Airport Blvd Srvc Rd/Popeyes Restaurant Work Zone):  On or about February 23, 2022, the employer failed to provide head protection for platform employee(s) operating a Wilkie Crane Truck (Model: 52XLR Serial No.: 2152XLR1736) while performing electrical service/maintenance operations; exposing employee(s) to crushing and collision hazards from the overhead sign flange and parts of the sign.
