29 CFR  1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   a) 7 Kingsville Drive, Monroe, NY - On or about February 14, 2022, an employee was operating a concrete pump hose while standing on top of concrete forms approximately 10-feet above the ground and was not protected from falling.

The employees did not received fall protection training.
